Job Title: Outreach Coordinator

Department: Council on Aging (COA)

Location: Town of Blackstone

Job Type: Full-Time

Salary: $20.48 - $27.29

About the Position:
The Town of Blackstone is seeking an experienced and compassionate Outreach Coordinator to join the Council on Aging (COA). This position is responsible for coordinating and implementing social services for senior and disabled adults, including identifying resources, providing client education and advocacy, making service referrals, and managing cases.

Key Responsibilities:

  • Assist senior and disabled residents with eligibility and applications for public and nonprofit assistance programs (e.g., fuel assistance, SNAP, MassHealth, Medicare, Social Security).
  • Maintain organized documentation of client interactions and data records.
  • Provide customer service through drop-in visits, scheduled appointments, home visits, and phone calls.
  • Research and maintain updated lists of relevant services and resources.
  • Collaborate with external organizations to coordinate care for residents and transmit documents promptly.
  • Assist in Senior Center operations, including program setup as needed.
  • Communicate with town departments (e.g., Board of Health, Fire, Police) for case referrals.
  • Promote outreach services by creating and distributing marketing materials.
  • Report suspected client abuse to the COA Director and document interactions with appropriate agencies.
  • Prepare reports for the town’s annual budget, including job statistics and outreach department calls.
  • Contribute monthly content to the Senior Center newsletter.

Qualifications:

  • Bachelor’s degree in social services or a related field, plus 1 year of relevant experience, or an equivalent combination of education, training, and experience.
  • Knowledge of elder care programs, social services, and the aging process (e.g., Alzheimer’s, diabetes).
  • Proficiency in office equipment and software, including MySeniorCenter, email, word processing, spreadsheets, and social media platforms.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to manage multiple priorities, exercise independent judgment, and maintain confidentiality.

Work Environment:

  • Majority of work performed in an office setting.
  • Physical demands include sitting, standing, walking, and occasionally lifting objects up to 30 pounds.
