Outreach Coordinator Town of Swampscott COA

The Town of Swampscott is seeking a full-time Outreach Coordinator.  

This compassionate and community-oriented individual will provide assessment, information and referral, advocacy, case management, supportive consultation, and community resource coordination to older adults, caregivers, and individuals with disabilities. The position assists residents in accessing programs, benefits, and services that promote independence, dignity, health, safety, and quality of life. Working collaboratively with the Director of Aging Services, the Outreach Coordinator serves as a trusted resource for residents and families navigating social, emotional, financial, housing, healthcare, transportation, caregiving, and aging-related challenges.

Key responsibilities Include:

Assessment, Case Management, and Advocacy 

• Conduct comprehensive psychosocial assessments to identify residents' strengths, needs, risks, and available supports. 

• Conduct home visits, office appointments, virtual meetings, and telephone consultations as appropriate. 

• Provide administrative and technical support for transportation services, document day-to-day scheduling, track changes, and document program usage.  

• Provide information, referrals, advocacy, supportive consultation, emotional support, and case management services. 

• Assist residents with accessing healthcare, housing, transportation, nutrition, financial assistance, public benefits, caregiver resources, veterans' services, and other community programs. 

• Assist residents with completing applications and eligibility documentation for local, state, and federal assistance programs. 

 • Maintain confidential client records in accordance with professional standards, HIPAA, and applicable state and federal laws. 

• Respond to crisis situations involving housing instability, caregiver stress, social isolation, mental health concerns, or other emergencies requiring immediate intervention. 

• Coordinate referrals to Protective Services when concerns involving abuse, neglect, self-neglect, or financial exploitation are identified.

Outreach and Community Engagement 

• Conduct proactive outreach to isolated, vulnerable, and at-risk older adults and caregivers. 

• Identify residents who may benefit from Council on Aging services and community resources. 

• Build and maintain collaborative relationships with hospitals, healthcare providers, elder service agencies, housing authorities, veterans' organizations, behavioral health providers, faith-based organizations, and other community partners. 

• Participate in community outreach events, health fairs, educational programs, wellness initiatives, and public awareness campaigns. 

• Assist clients in determining activities that match their interests and needs. 

• Maintain current knowledge of community resources, public benefits, and emerging issues affecting older adults and caregivers. 

Dementia Support, Caregiver Services, and Specialized Programming 

• Coordinate and facilitate caregiver support groups and educational programs. 

• Coordinate and facilitate Memory Café programming and dementia-friendly initiatives. 

• Provide education and support to individuals and families affected by Alzheimer's disease and other forms of dementia. • Assist families with dementia care planning, community resources, caregiver education, and long-term support options. 

• Co-facilitate the weekly By the Sea Social Day Program, a four-hour supervised adult day respite program, in collaboration with the Director of Aging Services and the Program and Volunteer Manager. Responsibilities include planning and implementing engaging activities appropriate for participants living with early to mid-stage memory loss.

Internship Coordination 

• Assist with coordination of the Salem State School of Social Work internship program. 

• Provide orientation, mentoring, if sufficiently credentialed provide field supervision, review of process recordings, and completion of mid-year and final learning evaluations for assigned graduate-level social work interns. 

• Support interns in developing professional practice skills while ensuring compliance with program requirements.  

Minimum Qualifications

• Bachelor's degree in Social Work, Human Services, Gerontology, Psychology, Sociology 

• Three (3) years of professional experience in aging services, social services, healthcare, behavioral health, case management, or a related field. 

• Valid Massachusetts Class D Driver's License. 

Preferred Qualifications

• Master's Degree in Social Work (MSW) or a closely related field. 

• Massachusetts Licensed Certified Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), or eligibility for licensure. 

• Experience working with older adults, caregivers, and individuals living with dementia. 

• Experience conducting assessments, crisis intervention, care planning, and community-based case management. 

• Experience supervising interns or mentoring students is desirable
