Outreach / Support Services Coordinator, Council on Aging: Performs administrative, clinical and supervisory work in managing the coordination of social services to the Town’s older adults; including, community outreach, resource counseling, coordination of support groups, information and referral services, related administrative work, and all other related work as required. Bachelor's Degree in social work, sociology, psychology, or gerontology; two to four years of experience in working with the elderly in a social service capacity; or any equivalent combination of education and experience. Licensed social worker preferred. Supervisory experience and knowledge of Cape social service network strongly preferred. Thorough knowledge of gerontology, the needs of older adults and caregivers, social services, and the health and human service network for seniors. Thorough knowledge of public assistance and benefit programs such as Medicare, Medicaid, Social Security Insurance/Social Security Disability Insurance, Long term Care Insurance, Health Safety Net, Mass Health. Knowledge of rules and regulations for financial matters, as they pertain to older adults (RE Tax abatement; Fuel Assistance, etc.); Knowledge of appropriate referral sources in areas of housing, home health care, medical insurance, and financial and legal matters. Ability to assess the needs of the older adults and caregivers and problem solve to coordinate appropriate services and programs to accommodate these needs. Ability to develop effective and constructive working relationships with older adults, caregivers, community resources, and the general public. Ability to work capably with confidential and sensitive information and adhere to strict confidentiality guidelines. Ability to maintain composure and provide leadership in crisis situations. Ability to exhibit professional initiative. Ability to communicate appropriately with town officials, departmental employees, the general public, and especially with the older adult population. Ability to prioritize work and perform multiple tasks in a detailed and organized manner. Ability to maintain accurate and detailed records. Strong counseling, management, leadership, supervisory, customer service, and public relations skills. Proficient and effective written and oral communication skills. Considerable judgment in the handling of individual problems and issues. Compassion for older adults. Proficiency in the use of computers including skills in word processing, spreadsheets, data input, and publication software. Respectful, empathetic, courteous, sensitive, patient, and flexible. 

$2,327.97 biweekly + benefits. Open to the Public until 10/29/24.
